How to Avoid Common Errors on Physical Scripts - For Prescribers and Clinics

Check these details are on the physical FP10 prescription before you post it to the pharmacy.

Required details on the script

Scripts written incorrectly are a common issue. Before posting the physical FP10, check it includes:

  • Patient's full name

  • Patient's address

  • Prescriber's name

  • Prescriber's address

  • Prescriber's six-digit private prescriber identification number

🗒️ Note: This number is issued by your local NHS England regional team when you register as a private prescriber. It's not your GMC or GPhC number, and it's separate from the PIN patients use for secure delivery handover.
