Required details on the script
Scripts written incorrectly are a common issue. Before posting the physical FP10, check it includes:
Patient's full name
Patient's address
Prescriber's name
Prescriber's address
Prescriber's six-digit private prescriber identification number
đď¸ Note: This number is issued by your local NHS England regional team when you register as a private prescriber. It's not your GMC or GPhC number, and it's separate from the PIN patients use for secure delivery handover.
